Our rehabilitation programs are comprehensive and include patient education, supervised exercise and support. Our team of healthcare professionals will work with you to develop a rehabilitation plan and monitor your progression through the program. 

Program Goals
■ Identify, modify and manage risk
    factors to reduce disability,
    morbidity and mortality.
■ Improve functional capacity.
■ Alleviate/lessen activity-related
    symptoms.
■ Educate patient about management of disease.
■ Improve quality of life.

Core Program Components
■ Risk factor management.
■ Baseline and ongoing patient assessment.
■ Monitored progressive exercise/activity
■ Medically supervised program.
■ Lifestyle modification.
■ Individualized treatment – typically three times per week for up to 12
    weeks.
■ Evidence-based practice guidelines.
■ Patient self-management education.
■ Process and outcomes measurement, evaluation and management.
■ Routine reporting back to the physician
