Disease Management Strategies
St.
John’s Medical Management is dedicated to helping people live
healthier lives. If your employees have been diagnosed with a chronic
disease or are pregnant, the St. John’s Disease Management Program can
help them lead healthier lives and avoid costly trips to the emergency
room or hospital. Highly skilled nurses and clinicians reach out to
help educate and motivate individuals to self-manage. Individual
counseling, assessment and group classes (in certain geographical
areas) are also available with these programs.
St. John’s Medical Management has partnered with your company to offer
individuals education, guidance, and tools to help them manage disease
