Individual gift
You can give a gift by check, money order or credit card.

Corporate gift
If you operate a business, you can make a gift on behalf of your company. Some
companies also develop matching gift funds to give employees an added incentive
to participate in charitable giving in their region.
¢Tribute gifts
Tribute gifts or memorials are given in honor and remembrance of special
people and significant occasions in life. Gifts may be made in honor of a loved
one, a nurse or physician. Gifts also may be made in memory of a loved one's
birthday, anniversary or a holiday such as Memorial Day. Tribute gifts may be
directed to any fund, and an acknowledgement of the gift will be sent to the
family of the person being remembered or to the person being honored.
¢ Create a living trust or annuity
Consider naming St. John's Foundation for Community Health as the beneficiary
to your living trust or create a charitable gift annuity. Both options will pay
an income for life to you or a loved-one and benefit the Foundation.

Giving through life insurance
There are several ways to give a gift to the St. John's Foundation for
Community Health through existing life insurance policies you may own.

Stocks and other highly appreciated assets
You can give by way of transferring stocks and other assets or investments you
currently own.

Giving through a will
This is a good option for people who are not able to give immediately or for
people want to continue to help make a difference for future generations.
Individuals that have included St. John's Foundation for Community Health in
their will are recognized as members of the St. John's Legacy Society.

Unrestricted
These funds are used to support our greatest needs.

Designated
These funds are used to support specific areas or institutions as determined
by the benefactor (i.e. Mercy Villa, Nursing Scholarships).

Fields of Interest
These funds are used to support general health-related areas as determined by
the benefactor (i.e. health care for uninsured/underinsured).
¢
Donor Advised
These funds are used to support an area or institution as determined by the
benefactor on an annual basis. This allows a benefactor to support a different
area each year.
