Grants
Since 2001, St. John’s Foundation for
Community Health has provided more than $8.7 million in assistance for St. John’s programs and
community projects. Grants funded to the community are made possible by
charitable distributions from unrestricted funds that have been established
within the Community Foundation of the Ozarks.
Please remember that St. John’s Foundation for
Community Health seeks proposals that are consistent with the mission, vision
and strategic initiatives of St.
John’s Health System.

St. John’s is a
fully integrated health system (hospitals, physicians and health plans) which
is committed to improving health status, strives to exceed the expectations
of customers and contracts its services to the community.
Mission
Rooted in the Mission of Jesus and the healing ministry of the church, our
mission is to provide high quality, compassionate healthcare services in a
personalized Christian environment. We
seek to improve the health and quality of life pf communities we serve, with
particular concern for people who are economically poor.

Procedures – How to Apply
Please note:
application packages will be returned for failure to include the following
required documents.
Grant applicants must complete the application.
Organizations should include the following with the grant
application.
·
Current letter indicating 501(c)(3) status.
·
A project budget summary including all other funding or future
support.
·
A list of the organization’s board members and trustees.
·
Organizations mission statement.
·
Please attach any project brochures or marketing materials
·
A written proposal not to exceed two pages and must include:
1. State whether this is a new or
ongoing project for the organization.
2. Description of the unmet
need. How will this project address
the need? What is the target population and how will the population benefit
from the program?
3. Describe project goals and
objectives. How are they measured?
4. Explain how the program or service
will be sustained beyond Foundation funding.
What other grants are you applying for? What other fund are secured fro this
project?
5. How does this project meet and
match St. John’s
mission, vision and strategic initiatives?
