St. John’s Home Care is available to
help patients make an easy and safe transition from hospital care
to home care, and ultimately a quick return to their normal
lifestyle.
We offer several programs designed to meet patient’s
home care needs. Each program is dedicated to providing the best
patient care available.
St. John’s Home Care provides
quality, compassionate care through trained homemakers, nurse
aides, LPNs, RNs, physical therapists, occupational therapists,
speech pathologist and medical social worker services. Home care
services are available 24-hours a day, seven days a week. It may
be provided on a continuous or intermittent basis depending on the
needs of the patient or caregiver at home and also includes
pediatric care.

St. John’s Home Health provides physician ordered medical services
to patients in their home. These services are available to
patients of all ages. Our nurses provide home visits for
assessments, wound care, post-surgical care and Instruction in self care.
Our pediatric nurses provide
special care for infants, children and adolescents, including
immunizations, wound care and infusions.
In addition to the medical services
described above, St. John’s Home Health offers physical, occupational and
speech therapies. Home
health aides and medical social workers are also available to
provide assistance.

Hospital stays can often be shortened or avoided by administering
IV medications in the home. St. John’s Home Infusion staff can
safely provide therapies such as antibiotics, chemotherapy, pain
management, hydration and TPN (nutrition administered
intravenously) in the home.
Our specially trained nurses and
pharmacists will provide all the medications, equipment and
supplies needed to successfully complete the therapy. Our staff
works with both patients and caregivers to provide instruction
about operating the equipment and caring for the IV line.

Patients who are in the end stage of their lives have special
medical, spiritual and emotional needs. St. John’s HospiceCare
provides a variety of services designed to meet these special
end-of-life needs.
Services are provided in the
patient’s home and are coordinated through a team of dedicated
professionals, including the patient’s physician, experienced
nurses, home health aides, social workers, chaplains and trained
volunteers.
The focus of hospice care is to
provide comfort, care and support in an effort to enable
end-of-life patients to live life to the fullest extent possible.

St. John’s In Home Services provides respite care, homemaker care,
basic personal care and advanced personal care services.
Some of these services include Meal
preparation, grocery shopping, cleaning, laundry, bathing and
dressing, toileting and advanced personal care such as care of
ostomies, catheters and more.
St. John’s In Home Services also
provides adult private duty nursing services covered by insurance
or private pay.
