St. John’s Senior Health Center is a
primary care clinic dedicated to improving the quality of life
for seniors on Medicare and Medicare beneficiaries.
The key to the St. John’s Senior
Health Center services is to provide coordinated care. We have
one internal medicine physician experienced in senior care, a
board-certified geriatrician, a social worker, nurses and support
staff all experienced in senior health.
Our staff strives to become
familiar with the total picture of each individual’s health needs
and coordinates their care to make sure those needs are met in
the best way possible.
We are able to spend the
extra time required by our patients and provide support services for
families and caregivers. We are also able to help our patients with life
planning.

In addition to a regular office visit, we
provide a nutritionist and pharmacologist in our office on a
weekly or as-needed basis for all of our patients.
